Key Responsibilities

1. Medication Management:
   • Provide pharmaceutical care to patients by reviewing and interpreting physician orders, detecting therapeutic incompatibilities, and counseling patients on drug therapy.
   • Prepare, dispense, and label medications accurately and efficiently.
   • Monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose.
2. Clinical Services:
   • Participate in ward rounds with the multidisciplinary healthcare team.
   • Provide drug information and recommendations to physicians, nurses, and other healthcare professionals.
   • Collaborate with the healthcare team to develop and implement medication therapy management plans.
3. Education and Training:
   • Provide education and counseling to patients regarding their medication therapy, side effects, and interactions.
   • Conduct in-service training for healthcare professionals on topics related to pharmacotherapy.
4. Quality Assurance and Compliance:
   • Ensure compliance with applicable laws, regulations, and standards related to pharmacy practice.

Qualifications and Experience

• Bachelor’s or Master’s Degree in Pharmacy from a recognized institution.
• Registered Pharmacist with a valid pharmacy license.
• Good knowledge of pharmaceutical care principles and practices.
