Responsibilities:
- Provide personal care and support to individuals in accordance with their care plans.
- Assist with daily living activities, including dressing, bathing, grooming, and meal preparation.
- Support individuals in maintaining their physical and mental well-being, promoting independence and autonomy.
- Engage in meaningful activities with individuals, fostering social interaction and emotional support.
- Administer medication and ensure proper documentation as per guidelines and individual needs.
- Support individuals in accessing community resources and activities, promoting their inclusion and integration.
- Monitor and record individuals’ progress, changes in behavior, and any incidents or concerns.
- Collaborate with the care team to develop and implement personalized care plans.
- Communicate effectively with individuals, their families, and external healthcare professionals.
- Uphold confidentiality and respect individuals’ privacy and dignity.
- Follow health and safety protocols and report any issues or hazards promptly.
Requirements:
- Previous experience as a Support Worker or in a similar role is preferred but not essential. Relevant training or qualifications are advantageous.
- Compassion, empathy, and a genuine desire to make a positive difference in individuals’ lives.
- Excellent communication and interpersonal skills, with the ability to build rapport and establish trusting relationships.
- Ability to work effectively as part of a team and collaborate with colleagues and external professionals.
- Understanding of person-centered care and the ability to support individuals’ choices and preferences.
- Basic knowledge of health and safety regulations and safeguarding procedures.
- Flexibility to work shifts, including mornings, evenings, weekends, and overnight stays, as required.
- Physical stamina and the ability to assist with personal care tasks.
