Responsibilities:

  1. Support and Care Provision: Provide personal care and support to individuals with physical and/or mental health conditions, disabilities, or elderly individuals in need of assistance. This includes tasks such as dressing, bathing, toileting, feeding, and administering medication, while promoting dignity and respect.
  2. Individualized Care Plans: Assist in the development and implementation of individual care plans in collaboration with service users, their families, and other healthcare professionals. Ensure that care plans are tailored to meet the specific needs and preferences of each individual.
  3. Emotional Support: Offer emotional support, companionship, and a listening ear to individuals, fostering positive relationships and promoting their well-being. Provide opportunities for social interaction and engagement in meaningful activities.
  4. Risk Assessment: Conduct regular risk assessments to identify potential hazards and ensure the safety of service users. Take appropriate measures to minimize risks and report any concerns to the relevant authorities.
  5. Documentation and Reporting: Maintain accurate and up-to-date records of all care provided, observations, and any significant incidents. Report any concerns or changes in the service user’s condition to the appropriate team members promptly.
  6. Liaison and Collaboration: Work collaboratively with other members of the care team, including social workers, nurses, and other healthcare professionals. Communicate effectively and share information to ensure the delivery of holistic care.
  7. Professional Development: Engage in continuous professional development activities to enhance knowledge and skills in social care. Stay informed about best practices, policies, and regulations governing social care services.

Requirements:

  • Previous experience in a similar role within social care or healthcare settings is desirable but not essential.
  • Empathy, compassion, and a genuine desire to improve the lives of vulnerable individuals.
  • Excellent interpersonal and communication skills, with the ability to build rapport and establish positive relationships.
  • Good organizational and time management skills, with the ability to prioritize tasks effectively.
  • Understanding of the principles of confidentiality and the importance of maintaining privacy.
  • Ability to work independently and as part of a team, demonstrating flexibility and adaptability.
  • Willingness to work variable hours, including weekends and evenings, as required.
