Job Summary:

Care Sante is currently seeking compassionate and dedicated individuals to join our team as Community Care Assistants. As a Community Care Assistant, you will provide essential support and care to individuals in their own homes, assisting them with daily tasks and promoting their independence. You will play a vital role in improving the quality of life for our clients and ensuring their well-being.

Responsibilities:

1. Provide personal care and assistance to individuals in their homes, respecting their dignity, privacy, and cultural preferences.
2. Assist clients with daily activities such as bathing, dressing, grooming, toileting, and mobility.
3. Administer medication as per clients’ care plans, ensuring accuracy and adherence to protocols.
4. Prepare and serve meals and assist with feeding if necessary, considering any dietary requirements or restrictions.
5. Support clients with light housekeeping tasks, including cleaning, laundry, and maintaining a safe living environment.
6. Escort clients to medical appointments, social outings, and community activities, ensuring their safety and well-being.
7. Monitor and report changes in clients’ physical or mental health conditions to the appropriate healthcare professionals.
8. Offer companionship and emotional support, engaging in meaningful conversations and activities with clients.
9. Maintain accurate and up-to-date records of care provided, observations, and any incidents or concerns.
10. Collaborate with other healthcare professionals, family members, and external agencies to ensure coordinated and holistic care for clients.

Requirements:

1. Prior experience in a similar role, preferably in home care or a healthcare setting.
2. Compassionate and empathetic nature, with a genuine desire to help and support others.
3. Basic understanding of personal care techniques and familiarity with assisting individuals with mobility and personal hygiene needs.
4. Knowledge of medication administration procedures and the ability to follow care plans accurately.
5. Excellent communication skills, both verbal and written, with the ability to interact effectively with clients, their families, and healthcare professionals.
6. Ability to work independently and prioritize tasks in a dynamic and ever-changing environment.
7. Reliability and flexibility to work various shifts, including evenings, weekends, and holidays.
8. Valid driver’s license and access to a reliable vehicle for transportation purposes.
9. A commitment to maintaining confidentiality, professionalism, and adherence to ethical standards.
10. Relevant certifications or training in social care, healthcare, or personal support are desirable.
