Responsibilities:

  • Manage and supervise a team of home care workers, including nurses, aides, and support staff.
  • Coordinate and schedule home care visits for clients based on their needs and care plans.
  • Conduct assessments of clients’ needs and develop individualized care plans.
  • Monitor the delivery of home care services and ensure compliance with regulatory requirements.
  • Provide guidance and support to home care staff, including training and performance management.
  • Collaborate with other healthcare professionals and community agencies to coordinate care and support services.
  • Evaluate the effectiveness of home care programs and implement improvements as needed.

Education and Qualifications:

  • Bachelor’s degree in nursing, social work, or a related field.
  • Registered Nurse (RN) or Registered Social Worker (RSW) designation is preferred.
  • Previous experience in home care management or a similar supervisory role is an asset.
  • Knowledge of home care policies, procedures, and regulations.
  • Strong leadership and interpersonal skills.
  • Excellent organizational and communication skills.
