Job Description

We are seeking a dedicated and compassionate Practice Nurse to join our healthcare team in the United Kingdom. As a Practice Nurse, you will play a vital role in delivering high-quality primary care services to our patients within a general practice setting. You will work collaboratively with the medical team to provide comprehensive nursing care, health promotion, and disease prevention services.

Responsibilities:

  1. Patient Care: Provide high-quality nursing care to patients of all ages, including assessment, treatment, and management of acute and chronic conditions. Administer medications, perform vaccinations, wound care, and other clinical procedures as required.
  2. Health Promotion and Disease Prevention: Deliver health education, advice, and support to patients, promoting healthy lifestyles and disease prevention strategies. Conduct health screenings, including blood pressure checks, BMI assessments, and health assessments for specific conditions.
  3. Chronic Disease Management: Assist in the management and monitoring of patients with chronic conditions such as diabetes, asthma, hypertension, and COPD. Provide education, self-management support, and assist in developing care plans.
  4. Immunizations and Vaccinations: Administer routine immunizations and vaccinations according to national guidelines and protocols. Maintain accurate immunization records and ensure appropriate vaccine storage and handling.
  5. Collaborative Care: Work closely with the general practitioner, practice nurses, and other healthcare professionals within the practice to coordinate patient care, referrals, and follow-ups. Participate in multi-disciplinary team meetings and contribute to care planning.
  6. Documentation and Record-keeping: Maintain accurate and up-to-date patient records, including medical history, treatment plans, and consultations. Ensure confidentiality and adherence to data protection regulations.
  7. Infection Control and Health and Safety: Follow infection control protocols and maintain a safe and clean clinical environment. Adhere to health and safety policies and procedures, including safe disposal of sharps and hazardous waste.
  8. Continuing Professional Development: Stay updated with current nursing practices, guidelines, and evidence-based research. Attend relevant training, workshops, and conferences to enhance clinical knowledge and skills.
  9. Quality Improvement: Contribute to quality improvement initiatives within the practice, including clinical audits, patient satisfaction surveys, and implementation of best practices.
  10. Patient Engagement and Support: Foster effective communication and build trusting relationships with patients. Provide emotional support, guidance, and reassurance to patients and their families.

Requirements:

  1. Registered Nurse qualification with an active registration with the Nursing and Midwifery Council (NMC) in the UK. Practice Nurse experience is desirable but not essential.
  2. Strong clinical skills with the ability to assess, diagnose, and treat common acute and chronic conditions.
  3. Knowledge of health promotion, disease prevention, and management of long-term conditions.
  4. Competence in performing clinical procedures such as wound care, vaccinations, and venepuncture.
  5. Excellent communication and interpersonal skills, with the ability to engage and build rapport with patients of diverse backgrounds and ages.
  6. Good organizational skills and the ability to manage time effectively in a fast-paced environment.
  7. Familiarity with clinical information systems and proficiency in computer literacy.
  8. Commitment to patient-centered care, ethical practice, and professional development.
  9. Awareness of clinical governance and quality improvement principles.
  10. Ability to work autonomously as well as collaboratively within a multidisciplinary team.
