Key Responsibilities:
- Dementia Care: Deliver expert dementia care and support to patients admitted to the Urgent Care Unit, tailoring interventions to meet individual needs and ensuring the highest standard of care at all times.
- Crisis Management: Respond promptly and efficiently to dementia-related crises, offering support to patients and their families while collaborating with the interdisciplinary team to implement appropriate care plans.
- Family Support: Offer guidance, emotional support, and practical advice to families, carers, and loved ones, helping them cope with the challenges of dementia and assisting with decision-making during emergencies.
- Communication: Facilitate effective communication between patients, families, and the healthcare team to ensure everyone is well-informed about the patient’s condition, progress, and treatment plan.
- Education and Training: Provide training and education to healthcare staff on dementia-related issues, ensuring they have the necessary knowledge and skills to care for patients with dementia effectively.
- Advocacy: Advocate for the needs and rights of patients with dementia, ensuring they receive equitable access to appropriate healthcare services and resources.
- Record Keeping: Maintain accurate and up-to-date records of patient assessments, care plans, and interventions in compliance with relevant policies and procedures.
- Continuous Improvement: Engage in ongoing professional development, keeping abreast of the latest advancements in dementia care and implementing best practices in the field.
Requirements:
- Registered Nurse (RN) with valid registration and licensure to practice in the UK.
- Post-graduate qualification in Dementia Care or related field is highly desirable.
- Proven experience in providing dementia care, preferably in an urgent care or acute setting.
- Strong understanding of the complexities of dementia and its impact on patients and families.
- Excellent communication and interpersonal skills to engage effectively with patients, families, and multidisciplinary teams.
- Ability to remain calm and composed in high-pressure situations, providing effective crisis management.
- Commitment to delivering patient-centered care with empathy, compassion, and respect.
