Key Responsibilities:
- Client Assessment: Conduct thorough assessments of clients’ physical, emotional, and cognitive needs to develop personalized care plans.
- Care Coordination: Create and manage individualized care plans, including assistance with activities of daily living, medication management, and specialized care if needed.
- Client and Family Communication: Maintain regular communication with clients and their families to ensure their needs are met and to address any concerns or changes in care.
- Staff Supervision: Supervise and support a team of caregivers, providing training, guidance, and performance evaluations.
- Quality Assurance: Ensure that care services meet or exceed industry standards and regulatory requirements.
- Scheduling: Coordinate caregiver schedules to ensure proper staffing for client needs.
- Client Safety: Implement safety protocols and respond to emergencies effectively.
- Documentation: Maintain accurate and up-to-date client records and documentation.
- Community Engagement: Build relationships with local healthcare providers and community organizations to enhance support for clients.
Qualifications and Requirements:
- Bachelor’s degree in Nursing, Social Work, or a related field. Master’s degree is a plus.
- Registered Nurse (RN) or Social Work (SW) license, if applicable.
- Proven experience in care management, preferably in home care or senior care.
- Strong understanding of aged care regulations and standards.
- Excellent communication and interpersonal skills.
- Leadership and team management abilities.
- Compassion and empathy for seniors and their families.
- Proficiency in relevant computer software for care management.
